Margarita Hart serves as the Executive Director of both INCHWA and Esperanza Ministries, bringing over three decades of international and domestic experience in the Community Health Worker (CHW) field. A lifelong CHW, Margarita is an ICF-accredited Professional Certified Coach (PCC), a Board-Certified Chaplain, and a Certified Community Health Worker. She is also a Master Neurolinguistic Practitioner with a specialized focus on transformational leadership development for CHWs—supporting professionals from frontline service to executive leadership.
Margarita is actively engaged in policy development at both the state and national levels, advocating for systemic improvements that strengthen and sustain the CHW workforce. Her contributions as an author of several white papers have influenced policy and best practices impacting community-based care, workforce equity, and culturally responsive public health models.
With deep expertise in culturally responsive coaching, ethical leadership, and strategic organizational consulting, Margarita empowers both individuals and institutions to foster sustainable, community-rooted change. As a multilingual interpreter, dynamic speaker, and published author, she equips emerging leaders with the tools and mindset needed to lead with authenticity, clarity, and purpose.

Carey Frazier, CCHW, HCI-CLC, HCI-CHC
Assistant Director of INCHWA, Carey Frazier is a Certified Community Health Worker with over 20 years of experience serving and uplifting her community. With a deep-rooted passion for advocacy and workforce development, she plays a vital leadership role in advancing the CHW profession across Indiana.

In her role at INCHWA, Carey oversees specialty training curriculum development, membership engagement and care, and serves as the organization’s training vendor administrator. She is a driving force behind CHW advocacy efforts and serves as INCHWA’s marketing specialist—effectively communicating the latest best practices, resources, and opportunities to the workforce.

As a certified life and health coach (HCI-CLC, HCI-CHC), Carey brings a holistic, strengths-based, and action-oriented coaching approach. She empowers individuals to manage stress, build resilience, and align their personal and professional goals with their values.
Known for her supportive presence and strategic mindset, Carey creates inclusive spaces where growth can flourish—equipping CHWs and emerging leaders to lead with clarity, confidence, and purpose.

Jasmine D. Gonzalvo, PharmD, MPH Candidate, CDCES, FADCES

PharmD is Chris and Theresa Dimos, Director of the Center for Health Equity and Innovation, and clinical professor at the College of Pharmacy at Purdue University. She is pursuing a master’s in public health from the Harvard T.H. Chan School of Public Health with an expected graduation date of May 2025.
She provides cardiovascular risk reduction medication management services for diabetes, hypertension, dyslipidemia, and tobacco use disorders at Eskenzi Health, Pecar Health Center for a predominantly Latino/Hispanic population in a Federally Qualified Health Center.
She has served on and chaired national Boards for diabetes organizations. She is currently on the Board of Directors of the Diabetes Leadership Council and the Indiana Community Health Workers Association. She is actively in advocacy efforts at the state and federal levels, most recently having served on the National Clinical Care Commission, which is charged with making recommendations to improve diabetes across federal programs.
Her research interests relate to community health workers, cardiovascular risk, risk reduction in under-resourced populations, integration of the Spanish language into practice and academic settings, diabetes, self-management education, and support in the pharmacy setting. She has received various rewards highlighting her excellence in training and teaching.

Yumary Ruiz, PhD, MPH
Dr. Yumary Ruiz is an Associate Professor of Public Health and Associate Director of the Center for Rural and Migrant Health at Purdue University. Her research focuses on health disparities, behavior change, and the social determinants impacting migrant and mobile populations—particularly rural Latino/a youth from farmworker families and those facing housing instability.
Using community-based participatory research (CBPR), Dr. Ruiz partners with organizations to develop and evaluate programs that address the health effects of socioeconomic disadvantage. She is co-Principal Investigator of the Purdue Puentes Project, an NIH-funded longitudinal study on resilience among Latino/a youth, and co-Founder of Purdue’s Community Health Workforce Development Institute (CHWDI), where she advances CHW workforce models to improve access and community capacity in health disparities research.

Ryan I. Logan, PhD, MPH
Ryan is an Associate Professor of Medical Anthropology at California State University, Stanislaus. Logan collaborated with INCHWA on a long-term ethnographic research project with and for community health workers in Indiana from 2016-2023. He has served on the INCHWA Board since 2022 and is currently serving as Secretary. His research interests include medical anthropology, applied anthropology, public health, health equity, and community health workers.

Dr. Lisa Staten
Dr. Lisa Staten is an Associate Professor in the Department of Community and
Global Health at the Indiana University Indianapolis Fairbanks School of Public
Health. She has collaborated with community health workers/Promotoras as key partners in developing and implementing health promotion programs for more than 25 years.
The overarching theme of her work is improving health equity through community engagement. Before coming to Indiana, Dr. Staten was at the University of Arizona, where she worked with community partners to implement multilevel interventions to reduce the rates of diabetes in Latino communities along the US/Mexico border. She is the principal investigator of the Diabetes Impact Project – Indianapolis
Neighborhoods (DIP- IN), an eight-year project in partnership with three Indianapolis communities to target diabetes prevention and control.

Omolola (Lola) A. Adeoye–Olatunde, PharmD, MS
Dr. Omolola (Lola) Adeoye-Olatunde is the Darr-Chaney Assistant Professor of Pharmacy Practice at Purdue University and Founder of the HER Lab. Her research focuses on community-engaged, implementation science approaches to improving healthcare access in underresourced communities.

Through partnerships with local and national organizations—including projects funded by NIH and the Indiana Department of Health—she co-develops culturally tailored strategies to eliminate health disparities and transform healthcare delivery. Dr. Adeoye-Olatunde is a nationally recognized leader, receiving honors such as the Bowl of Hygeia Award, NIH Health Disparities Research Institute Scholar, and the AcademyHealth Alice S. Hersh Emerging Leader Award.

She holds a PharmD from MCPHS University, completed a PGY1 residency in community pharmacy, and earned an MS in Health Services, Outcomes, and Policy from Purdue University.

Dr. Natalia Rodriguez
Dr. Natalia Rodriguez is an Associate Professor of Public Health and Biomedical Engineering at Purdue University. She leads an interdisciplinary research group focused on the design and implementation of novel technologies to address health disparities. Partnering closely with CHWs to meaningfully engaging underserved communities in translational science, her lab employs participatory methodologies to increase adoption of technological innovation and to improve health outcomes for vulnerable populations.

McKenzie Reed, CCHW
McKenzie has worked as a Community Health Worker for CICOA Aging and In-Home Solutions since April 2022, and she was CICOA’s first CHW. She got her CHW certification in September 2022, and while working directly with clients, she supported them during their admission to and discharge from the hospital or nursing facility. As of March 2025, McKenzie serves CICOA as the Director of Community Health Programs, helping to manage and implement new and ongoing CHW interventions within the agency and creating a long-term sustainability plan, alongside the Healthcare Collaborations team. McKenzie was one of the INCHWA CHW Champion Award winners in 2024 and served on the INCHWA Advisory Board in 2024 before joining the Board.

Ashley Meredith
Dr. Ashley Meredith is a Clinical Professor in the College of Pharmacy at Purdue University in West Lafayette, Indiana.
Dr. Meredith serves as the Director of Advocacy for the Center for Health Excellence, Quality and Innovation (CHEQI) and as a Co-Director of the Community Health Workforce Development Institute (CHWDI) at Purdue University. Her work focuses on increasing access to care, with an emphasis on inclusion of CHWs as an integral part of these efforts.
